Resources & Information
If you or someone you know is experiencing a mental health crisis, help is available.
Crisis Support
988 Suicide and Crisis Lifeline

These teams can offer short-term treatment, psychiatric medication if needed, and arrange hospitalization under NYS Mental Hygiene Law 9.58 when appropriate.
Community-Based Mental Health Services (NYC)
New York City offers several free community mental health programs, many of which can be initiated through 988 or local providers:

· ACT (Assertive Community Treatment): Team-based, long-term mental health support for individuals with serious mental illness who struggle with traditional outpatient care.
· FACT (Forensic ACT): Intensive support for individuals with mental illness who are also involved in the criminal justice system.
· SPACT (Shelter Partner ACT): Provides ACT services to individuals in Brooklyn mental health shelters and nearby communities.
· IMT (Intensive Mobile Treatment): Offers comprehensive mental health and substance use services to individuals experiencing homelessness or frequent hospitalizations/incarcerations.

Medicaid and Financial Planning Resources
Medicaid Spend-Down
If your income is too high to qualify for Medicaid, the spend-down program allows you to contribute part of your income toward medical expenses to gain coverage.
This can help you access essential services like home health aides and long-term care.

Income Pooled Trust
An income pooled trust allows individuals with higher incomes to become Medicaid-eligible without spending their money on medical costs. The funds in the trust can be used for living expenses while maintaining Medicaid eligibility. CIDNY and other nonprofit legal services can help set up these trusts.
Difference between Spend-Down and Pooled Trust
· Spend-Down: You pay your medical bills until your income is low enough for Medicaid.
· Pooled Trust: You deposit excess income into a managed trust that pays for your expenses while maintaining Medicaid coverage.
